The Schuster Institute for Investigative Journalism at Brandeis University has published a report on the state of international adoption called, "Where Do Babies Come From: Corruption in International Adoption." From the report, "The story of abandoned orphans in developing countries who need to be whisked away to adoring moms and dads in faraway lands is, unfortunately, largely fiction." There is an associated article in Foreign Policy and a related map of countries with adoption irregularities and the countries that lead the world in adopting from international sources.
